Milia are epidermal inclusion cysts composed of keratin trapped beneath the stratum corneum. Because they sit under a thin, intact layer of skin, they do not connect to a pore and therefore cannot be squeezed like acne whiteheads. On close inspection, each milium appears as a tiny pearl‑white bead. They most commonly affect the eyelids and peri‑ocular skin, cheeks, nasal sidewalls and upper cheeks, but can occur anywhere—sometimes appearing on the chest, upper arms or genital skin following friction or procedures.
Dermatologists classify milia as primary (arising de novo) and secondary (following triggers such as blistering, burns, resurfacing, radiotherapy, or chronic sun damage). Recognising which pattern you have helps guide treatment and prevention.
Types of Milia
Primary milia
These form spontaneously from the outermost skin layer. They are common in adults and may cluster under the eyes and on the cheeks. In newborns, primary milia are extremely common (up to half of infants) and settle without treatment as the skin matures.
Milia en plaque
A rarer presentation in which numerous milia sit on an erythematous (pink) plaque—often near the ear, jawline, or eyelids. It can be persistent but responds to careful extraction, cautery or targeted laser.
Secondary (traumatic) milia
These follow skin injury or inflammation, including burns, friction from masks or sports eyewear, dermatitis, or aesthetic procedures such as ablative lasers. They may also appear along healed scars.
Eruptive milia

Diagnosis is typically clinical after a focused skin exam. Under magnification or dermoscopy, milia have a uniform, pearly appearance with a subtle white to yellow tone beneath a thin translucent surface. Your dermatologist will also consider differentials that can resemble milia:
- Syringomas: sweat‑duct tumours, often flesh‑coloured and clustered under the eyes.
- Sebaceous hyperplasia: yellowish papules with central indentation (commonly on the forehead).

Many milia in infants resolve spontaneously and require no treatment. In adults, persistent or cosmetically concerning lesions often need a precise, mechanical solution. We tailor the approach to location, skin tone, and lesion depth to minimise marks and pigment change.
1) Sterile Micro‑lancing & Extraction (Gold Standard)
For most facial milia—particularly around the eyes—micro‑lancing with a sterile, single‑use instrument followed by gentle expression is the safest and most predictable method. We prepare the skin, create a pinpoint opening on the surface, and release the keratin core with fine instruments. On sensitive areas we can use a topical anaesthetic cream.
