What Is Lichen Planus?

Lichen planus is a chronic inflammatory condition that affects the skin, and sometimes the scalp, nails and mucous membranes such as the mouth or genitals.

It is thought to be immune mediated. The immune system targets cells in the outer layer of the skin and mucosa, causing small itchy bumps, colour change or fragile, sore areas. The condition can be limited and mild, or more widespread and uncomfortable.

Lichen planus is not contagious and is not caused by poor hygiene. It often needs specialist assessment to confirm the diagnosis, rule out look-alike conditions and select safe, effective treatment options.

What Are the Symptoms of Lichen Planus?

Symptoms vary depending on where the lichen planus is affecting the body, but several patterns are typical.

  • Itchy, flat-topped, purple or reddish bumps on the wrists, ankles or lower back
  • Fine white lines or a network pattern on the surface of lesions
  • Dark marks where previous lesions have healed, especially in darker skin types
  • Scalp involvement leading to scaling or patchy hair loss (lichen planopilaris)
  • Nail changes, including ridging, thinning, splitting or nail loss
  • White streaks or sore patches in the mouth (oral lichen planus)
  • Sore, fragile or white patches in genital areas, sometimes overlapping with lichen sclerosus

Lichen planus can be localised to a small area or involve several body sites at the same time. A full skin, scalp, nail and mucosal review helps to build an accurate overall picture.

What Causes Lichen Planus?

Lichen planus is believed to be an immune-mediated condition. The immune system mistakenly targets cells at the junction between the outer and deeper layers of the skin or mucosa, leading to inflammation and damage.

In some patients, a similar eruption can be triggered by certain medications or by contact with specific substances, such as some dental materials. This is called a “lichenoid reaction” and may resolve when the trigger is addressed.

Other factors that can be relevant include:

  • Past or current medical conditions that need to be reviewed
  • Skin trauma or scratching (the Koebner phenomenon)
  • Possible overlap with other inflammatory or autoimmune conditions

Is Lichen Planus Serious?

Lichen planus can be very itchy and cosmetically distressing, but in many patients it can be controlled with appropriate treatment and careful follow-up.

Some forms, particularly longstanding mucosal disease (for example oral lichen planus or genital changes overlapping with lichen sclerosus), may carry a small risk of precancerous or cancerous transformation over time. This is why regular specialist review and prompt assessment of any persistent sore or thickened area are important.

Scalp and nail involvement, such as lichen planopilaris, can leave permanent scarring if not recognised and treated early. We therefore pay particular attention to scalp tenderness, hair loss and nail changes during your visit.

How Is Lichen Planus Diagnosed at Skinhorizon Clinic?

Lichen planus is usually diagnosed by an experienced dermatologist using a combination of careful clinical assessment and, where needed, a skin biopsy.

  • Consultant examination: your dermatologist will review your history and examine the skin, scalp, nails, mouth and genital areas sensitively and respectfully.
  • Dermoscopy: magnified examination of the skin helps to identify patterns typical of lichen planus and rule out other conditions.
  • Skin biopsy: a small sample of one or more lesions may be taken and analysed via our diagnostic skin biopsy service to confirm the diagnosis.
  • Blood tests: selected blood tests may be arranged through our dermatology-focused blood panels if clinically appropriate.

Where there is scalp, hair or nail involvement, additional evaluation with trichoscopy or nail assessment may be part of your overall work-up.

How Is Lichen Planus Treated?

Treatment for lichen planus aims to reduce inflammation, relieve symptoms and minimise the risk of scarring or long-term damage.
